Step 1 — Discover the config sets, read each checkpoint
`<set>` = one `notebooks/review/<repo>/`, sitting wherever `/open-pr:review` ran — a workspace keeps every repo's side by side, a repo reviewed from inside keeps its own. Search both, from pwd; FORBIDDEN: `cd`, deriving `<repo>` from a git remote (a workspace has none):

Each hit's subdirectories are the `<set>`s, named `<repo>`; key them by PATH — one `<repo>` may sit under 2 `notebooks/review/`, each with its own checkpoint.

Step 2 — Fetch the index, find versions newer than the checkpoint
`Read` `"${CLAUDE_PLUGIN_ROOT}"/core/llm-upgrades-index.md` for the fetch command + line grammar, run it, and collect every `N` strictly greater than the LOWEST checkpoint from Step 1 — 1 union for the run, so sets on differing checkpoints still cost 1 fetch.
None found → say the config is already current, name each `<set>`'s checkpoint, STOP. FORBIDDEN: fetching a `vN.md` to double-check — the index alone answers this.
**The index must never outrun the INSTALLED plugin.** Migrations arrive live ⇒ config could otherwise reach a shape the installed prompts misread, and every later review with it. Highest `N` > the checkpoint that same atom states ⇒ STOP before applying anything:

Step 3 — Fetch every matching `vN.md`
Each collected `N` → `llm-upgrades/vN.md`, same base URL, ALL in ONE batch. FORBIDDEN: fetch, wait, fetch — a later `vN` can override an earlier one's field, so nothing is decidable until all are in hand.
Step 4 — Summarise, then ask
ONE CHOICE per `core/guardrails.md`, EXACTLY 2 options, no hedging third. Its body NAMES every `<set>` — `<repo>`, checkpoint move, path when a `<repo>` repeats — so nothing is written against a repo the user never saw listed:
- `Upgrade all N (Recommended)` — detail: what changes per `ADDED`/`MODIFIED`/`REMOVED`/`RENAMED`, files touched
- `Not now` — detail: nothing written, every config keeps working
Subset wanted (free text, or a re-run naming them) ⇒ honour it, those only. FORBIDDEN: a SECOND question to pick repos — the list plus that argument cover it; step-by-step description; quoting `vN.md`. The user decides from WHAT changes, not HOW. `Not now` ⇒ STOP, nothing written.
Step 5 — Apply cumulatively, write the new checkpoint
Per `<set>`, apply migrations with `N` > ITS OWN checkpoint, ASCENDING — each `vN.md`'s instructions literally (fields added/modified/removed/renamed, files merged/split/renamed…). FORBIDDEN: assuming any target shape the fetched file does not state. Then set that `<set>`'s `schema_version` = highest `N` applied to it.
Step 6 — Report
Per `<set>`: which migration(s) ran and what changed, plainly — e.g. "merged `meta.json` + `fix-meta.json` into `settings.json`; config migration checkpoint 0 → 1" — plus any `<set>` dropped as never bootstrapped. Say **config migration checkpoint**, never "version": the plugin declares none, so a bare number reads as one. FORBIDDEN: dumping raw `vN.md` content or a JSON diff.
