Migration Numbering
Migrations follow strict sequential numbering:
- Format: `NNN_descriptive_name.sql` (e.g., `001_create_users.sql`)
- Each migration has two files: `NNN_name.up.sql` and `NNN_name.down.sql`
- Numbers are zero-padded to 3 digits
- Names use snake_case, describing the change concisely
